We are currently seeking an Intake Coordinator who will be responsible for processing patient referrals from a variety of referral sources, including but not limited to hospitals, doctor’s offices, rehabs, assisted living facilities. The Intake Coordinator will work directly with branches and leadership to timely and effectively coordinate patient care.  

Requirements

RESPONSIBILITIES:
  • Uphold Aveanna’s Core Values
  • Manage multi-line phone system, including answering and transferring calls
  • Follow up with referral sources timely
  • Enter referrals into the system
  • Address and resolve new patient intake issues
  • Generate and review reports (daily, weekly, monthly, as needed)
  • Prioritize tasks from supervisors and managers
  • Communicate effectively with all levels of the organization
  • Perform additional duties as assigned

REQUIREMENTS:

  • High school diploma/GED and college degree
  • Availability for after-hours on-call schedule
  • Proficiency in Microsoft Office and typing skills
  • Effective communication skills
  •  

QUALIFICATIONS:

  • Strong communication skills (telephone, email, in-person)
  • Skilled in data entry and process management
  • Self-motivated, results-oriented, and able to interact with all organizational levels
  • Strong analytical, judgment, and problem-solving abilities
  • Proficient with Microsoft Windows, Word, Excel, and PowerPoint
  • Professionalism and confidentiality in handling responsibilities
  • Ability to manage multiple competing priorities in a fast-paced environment
  • Client-focused with strong interpersonal skills for dealing with internal and external clients
  •
