Position Summary:
The Clinical Supervisor is a Registered Nurse (RN) responsible for leading, supervising, and supporting a team of clinical caregivers to ensure the delivery of high-quality, patient-centered care. This role includes overseeing care coordination, maintaining regulatory compliance, conducting supervisory visits, and guiding clinical staff development. The Clinical Supervisor plays a vital leadership role in ensuring patient safety, staff performance, and clinical excellence across home care services.
Essential Job Functions:
Care Coordination & Patient Oversight
Oversee the development and execution of goal-directed care plans for assigned patients from admission through discharge.
Perform comprehensive, ongoing assessments to ensure care plans remain responsive to patient needs.
Conduct regular in-home supervisory visits to assess patient condition, caregiver performance, and the home environment.
Provide direct patient care and clinical interventions as needed.
Ensure timely documentation and communication related to patient care and changes in condition.
Assist in securing re-authorizations for ongoing services and updating stakeholders accordingly.
Staff Supervision & Development
Provide direct supervision, orientation, training, and ongoing evaluation of clinical caregivers.
Participate in employment decisions, including hiring, coaching, and termination as appropriate.
Foster a positive, supportive work environment that promotes retention and professional growth.
Contribute to the development and implementation of performance improvement initiatives.
Compliance & Quality Assurance
Ensure adherence to all applicable clinical, regulatory, and accreditation standards, including CHAP.
Monitor and evaluate the quality and effectiveness of nursing services; identify opportunities for improvement.
Analyze clinical data and trends to inform risk mitigation and care optimization strategies.
Promote a culture of safety, ethical practice, and accountability.
Operations & On-Call Support
Ensure availability and proper functioning of necessary medical equipment and supplies.
Participate in the on-call rotation and respond promptly to patient and caregiver needs outside of normal business hours.
Collaborate effectively with internal teams and external partners to support seamless care delivery.
Communicate essential information clearly and consistently through proper reporting channels.
Qualifications:
Diploma, Associate, or Bachelor’s Degree in Nursing from an accredited RN program.
Active RN licensure in the applicable state(s).
Clinical experience as required by federal or state regulations.
Valid driver’s license and acceptable Motor Vehicle Record (MVR).
Preferred Qualifications:
Prior experience in a supervisory or leadership role within home health, private duty, or similar care settings.
Familiarity with administering and managing private duty nursing programs.
Skills & Competencies:
Strong clinical judgment and critical thinking skills.
Effective leadership, mentoring, and team-building abilities.
Proficient in time management, organization, and communication.
Knowledge of regulatory compliance and clinical documentation standards.
Ability to work compassionately with diverse populations, including families with limited resources.
Calm and professional demeanor in high-pressure or emergency situations.
Physical Requirements:
Ability to speak, read, write, and understand English.
Capable of lifting up to 40 pounds occasionally.
Prolonged standing, walking, bending, kneeling, reaching, and stair climbing.
Sufficient vision, hearing, smell, and touch to assess and care for patients.
Physical and mental capacity to respond effectively in emergencies.
Work Environment:
Must be able to work in varied home environments, potentially exposed to allergens and differing conditions.
Possible exposure to blood, bodily fluids, and infectious diseases.
