What You’ll Do

As a Clinical Supervisor, you’ll help ensure patients receive safe, high-quality care while serving as a clinical resource and leader for your team.

Responsibilities include:

• Coordinate care for assigned patients and develop individualized plans of care.

• Conduct supervisory visits in patients’ homes to assess patients, families, environments, and caregivers.

• Provide ongoing supervision, education, coaching, and support to clinical caregivers.

• Assess patients and provide direct patient care when needed.

• Collaborate with patients, families, physicians, caregivers, and other healthcare professionals.

• Monitor quality of care and identify opportunities to improve patient outcomes.

• Support staffing, care coordination, equipment needs, and authorization of continued services.

• Assist with hiring, onboarding, training, and performance management of caregivers.

• Promote caregiver engagement, satisfaction, and retention.

• Maintain compliance with company policies, professional standards, and applicable regulations.

• Participate in the on-call rotation and provide clinical support after hours as needed.

What We’re Looking For

Required:

• Diploma, Associate’s, or Bachelor’s degree in Nursing from an accredited program.

• Current RN license in the applicable state.

• Clinical experience as required by state and federal regulations.

• Valid driver’s license and acceptable MVR.

• Ability to travel throughout the assigned service area.

Preferred:

• RN experience in home care, private duty nursing, or a similar clinical setting.

• Experience supervising, mentoring, educating, or supporting healthcare professionals.

• Knowledge of private duty nursing and care coordination.

Physical & Environmental Requirements

This position requires travel throughout the assigned service area and the ability to perform essential clinical duties, including walking, standing, sitting, bending, reaching, climbing stairs, and occasional lifting/carrying of up to 40 pounds. The role may involve working in a variety of home environments and exposure to blood, bodily fluids, infectious diseases, allergens, and other conditions associated with patient care.
