ID/DD Coordinator

Aveanna Healthcare is seeking an ID/DD Coordinator to support individuals with intellectual and developmental disabilities and their families. In this role, you’ll help coordinate services, support individualized goals, collaborate with caregivers and care teams, and ensure services are delivered safely and in accordance with each individual’s needs and plan of care.

What You’ll Do

• Complete initial evaluations to assess client needs, appropriateness of services, and environmental safety.

• Participate in the development and implementation of Individual Support Plans (ISPs).

• Coordinate services based on each client’s individual needs, goals, and expected outcomes.

• Provide monthly written summaries of client progress and services to Supports Coordinators/Administrative Entities.

• Maintain effective communication with clients, families, caregivers, staff, and Supports Coordinators.

• Review paraprofessional progress notes to ensure appropriate and effective service delivery.

• Educate paraprofessional staff on individual client ISP expectations and service requirements.

• Monitor client status and remain informed of changes in medical, family, and service needs.

• Conduct employee performance evaluations at 90 days and annually.

• Maintain client confidentiality and professional boundaries at all times.

• Follow company policies, patient care standards, licensure requirements, and applicable state and federal regulations.

What We’re Looking For

Required:

• Bachelor’s degree in Psychology, Social Science, or a related field.

• Current CPR certification.

• Proof of eligibility to work in the United States.

• Current child abuse clearance and fingerprinting, as required.

• Ability to travel throughout the assigned service area; company vehicles are not provided.

Preferred:

• At least 1 year of experience working with individuals with intellectual and developmental disabilities (ID/DD).

Physical & Environmental Requirements

This position requires the ability to:

• Speak, write, read, and understand English.

• Travel to client locations throughout the assigned service area.

• Frequently lift, carry, push, or pull up to 25 pounds and occasionally up to 50 pounds.

• Walk, stand, bend, kneel, reach, and twist for extended periods as needed.

• Sit and climb stairs.

• Meet required visual and hearing acuity.

• Work in a variety of environments, including potential exposure to allergens, blood, bodily fluids, and infectious diseases.
